If the air side loses pressure or the diaphragm fails, the system has nowhere for the expansion to go. The pressure then rises too high when hot, can trigger the PRV, and you lose water each time the heating runs. The symptom you see is a big swing on the gauge and gradual loss of pressure.
4. Recently bled radiators without proper top-up
Bleeding radiators removes trapped air but it also lets water out of the system. If several radiators are bled and the system is not topped up correctly afterwards, the pressure may drop below the recommended level.
Often, homeowners notice that the upstairs radiators are cooler, bleed them, and then see the boiler pressure low the next day. A one-off top-up to the correct level is normal after bleeding, but you should not need to repeat this frequently.
5. Issues after topping up too often
If you keep adding water through the filling loop, you are not fixing the cause, only masking the symptom. Constant topping up can actually make things worse over time, as fresh water brings oxygen and minerals into the system.
